The history of Sri Lanka’s public welfare system is replete with milestones that have defined the nation as a beacon of social development in South Asia, but few achievements resonate as profoundly as the centenary of its community health service. This week, the island nation paused to honour a century of dedication, resilience, and clinical excellence during a grand commemorative ceremony held at the Bandaranaike Memorial International Conference Hall (BMICH) in Colombo. Under the distinguished patronage of President Anura Kumara Dissanayake, the event served not only as a retrospective of past triumphs but as a definitive launchpad for a modernised healthcare future, bolstered by a monumental government commitment of Rs. 45 billion.

The centenary celebration, meticulously organised by the College of Community Physicians of Sri Lanka (CCPSL), brought together a wide spectrum of stakeholders, from frontline health workers to high-level policymakers. The atmosphere at the BMICH was one of solemn reflection and renewed vigour, as the nation acknowledged the instrumental role played by the community health unit system in elevating Sri Lanka’s health indices to levels that often rival those of much wealthier nations. The presence of President Anura Kumara Dissanayake underscored the state’s recognition of healthcare as the foundational pillar of national stability and prosperity.

A Landmark Financial Commitment for the Future

The highlight of the proceedings was the significant policy announcement made by the Minister of Health, Dr Nalinda Jayatissa. Addressing the assembly of medical professionals and dignitaries, Dr Jayatissa revealed that the government has committed a staggering Rs. 45 billion towards the development and sustainment of community healthcare services over the forthcoming two-year period. This financial infusion is designed to fortify the primary healthcare network, ensuring that the preventive and promotive services which have been the hallmark of the Sri Lankan system remain robust amidst evolving demographic and epidemiological challenges.

This budgetary allocation is expected to facilitate the modernisation of Medical Officer of Health (MOH) offices across the island, the procurement of advanced diagnostic equipment for rural clinics, and the enhancement of training programmes for community health staff. By prioritising the “Arogya” health and wellness centres, the government aims to transition the national focus from reactive, hospital-based treatment to proactive, community-based prevention. This strategic pivot is considered essential for managing the rising burden of non-communicable diseases (NCDs) such as diabetes and hypertension, which currently place immense strain on the secondary and tertiary care sectors.

Honouring the Pillars of Primary Healthcare

Central to the centenary celebrations was the formal recognition of the human capital that drives the health service. Thirty-six community health professionals associations were honoured for their tireless contributions to the nation’s primary healthcare framework. These associations represent a diverse array of disciplines, including public health midwives, public health inspectors, community physicians, and nursing staff, all of whom operate as the primary interface between the state and the citizenry.

The ceremony served as a poignant reminder that while infrastructure and funding are vital, the efficacy of the community health service is fundamentally rooted in the trust and dedication of its personnel. Former President Maithripala Sirisena, who also attended the event, joined current leaders in acknowledging the historical continuity of these services, noting that the stability of the health system has remained a constant throughout various political and economic transitions in the country’s history.

The recognition of these associations highlights the collaborative nature of the Sri Lankan model. By integrating various professional cadres into a cohesive field-based unit, the system ensures that health interventions, ranging from maternal and child immunisation to environmental sanitation, are delivered directly to the doorsteps of families, regardless of their geographical or socio-economic standing.

Advocacy for Patients’ Rights

While the event was a celebration of institutional success, it also provided a platform for constructive dialogue regarding the future of the patient-provider relationship. Dr Vinya Ariyaratne, President of the Sarvodaya Shramadana Movement, utilized the occasion to urge the government to formalise and implement a comprehensive Patients’ Rights Charter. Dr Ariyaratne emphasized that as the healthcare system evolves, it is imperative that the dignity, autonomy, and rights of the individual remain at the forefront of policy.

A Patients’ Rights Charter would serve as a formalised framework to ensure transparency, informed consent, and equitable access to information within the health system. Such an initiative is seen as the antithesis of a purely bureaucratic health administration, fostering a culture of mutual respect and accountability. The proposal was met with considerable interest from the gathered professionals, reflecting a growing consensus that the next century of Sri Lankan health service must be as much about empowerment as it is about clinical intervention.

A Century of Progress: From Kalutara to the World

To appreciate the magnitude of this centenary, one must look back to the origins of the system in 1926. The establishment of the first Health Unit in Kalutara marked a revolutionary departure from the traditional colonial emphasis on curative, hospital-centric care. It introduced a systematic, field-based approach to public health that became a model for the entire South Asian region. Over the decades, this model expanded into the nationwide network of MOH divisions that today forms the backbone of the Ministry of Health’s preventive services.

The historical trajectory of the service has been defined by its ability to remain resilient during periods of national crisis, including the tsunami of 2004 and the global pandemic of 2020. In each instance, the community health network proved to be the country’s most effective tool for rapid response and community mobilisation. The commemorative publication launched at the BMICH documents these historical milestones, providing an academic and professional record for future generations of healthcare leaders.
